Registration set for DeQuincy Middle

DeQuincy Middle School will conduct new student registration Aug. 1-9 from 8:30 a.m. until 2:30 p.m. This registration is for any new 6th graders who did not finish the school year at DeQuincy Elementary and for 7th and 8th graders who are new to DeQuincy Middle School.

Parent or legal guardians must be present to enroll the student. If you are not the child’s parent, you must have legal documentation, from a judge, stating you have been given guardianship/custody of the student. Calcasieu Parish will no longer accept a notarized document.

To enroll the student you must bring the following information: Your child’s last report card with the name and address of their last school, current shot record, birth certificate and Social Security Card, and two proofs of address (bills in your name at your current address).

If you live with someone and don’t have utilities with your name or address, you must have the person you live with fill out an Affidavit of Address Verification form stating you live there and list everyone living with them. The form can be picked up at the school and must filled out completely and notarized.

You will also need to bring two proofs of their address. If your child has an IEP or is a special needs student, please bring a copy of their last evaluation and IEP with you. Failure to have the proper documentation will result in having to obtain a permit to register from the office of Child Welfare and Attendance for the Calcasieu Parish School Board, located at 2423 Sixth Street in Lake Charles.

Parents of brand new students to Calcasieu Parish are asked to log on to cpsb.org and under Quick Links on the left hand side, click on Online Registration Application. Complete each section and submit your registration application to save on paperwork when coming to the school. All information above must still be brought to the school as verification of guardianship, address, birth date and Social Security number.
